College Connect is built for emerging adults who need structure, accountability, and real‑world practice not generic encouragement or occasional check‑ins. Our model is hands‑on, relationship‑based, and designed to help young adults build the skills they need for college, work, and independent life. We don’t do the work for students. We help them learn how to do it themselves.
Step 1
Inquiry & Consultation
Families begin by reaching out to our team to discuss goals, strengths, concerns, and next steps.
We help determine whether College Connect is the right fit and outline the process for Regional Center funding when applicable.
What happens here:
• Initial conversation
• Overview of services
• Discussion of needs and goals
• Guidance on next steps




Step 2
Intake & Goal‑Setting
Once a student is ready to move forward, we complete a structured intake to understand their strengths, challenges, and long-term aspirations.
We gather information about:
• Academic history
• Executive functioning skills
• Social and communication needs
• Independent living skills
• Employment readiness
• Family goals and concerns
This becomes the foundation for the student’s individualized support plan.
Step 3
Individualized Support Plan
Every student receives a customized plan that outlines:
• Skill-building priorities
• Coaching frequency
• Academic or employment goals
• Independent living targets
• Communication expectations
• Progress markers
Plans are flexible and evolve as students grow.



Step 4
Ongoing Coaching & Monitoring
This is where the real work happens.
Students meet regularly with their coach to build systems, practice skills, and follow through on responsibilities.
Coaching may include:
• Executive functioning support
• Academic planning
• Employment readiness
• Social communication practice
• Independent living routines
• Real-world problem-solving
Coaches provide structure, accountability, and encouragement, always with the goal of increasing independence.
Step 5
Collaboration & Communication
We maintain appropriate communication with families, Regional Centers, and (when applicable) campus services to ensure aligned goals and consistent support.
Families receive:
• Updates on progress
• Guidance on supporting independence
• Clarity on expectations and next steps




Step 6
Growth, Independence & Next Steps
As students build skills and confidence, coaching shifts to support higher levels of independence.
Many students expand their goals over time — transferring schools, gaining employment, building friendships, or taking on new responsibilities.
Our goal is not lifelong dependence.
Our goal is lifelong capability.
